선천성 후두 폐색은 매우 희귀한 선천성 기형으로, 산전에 미리 진단되지 않으면 거의 대부분 출생 후 수분 내에 호흡부전으로 사망하게 된다. 후두 폐색은 기관 폐색, 그리고 기관 낭종 혹은 기관 망(web) 등과 함께 선천성 상기도 폐쇄(CHAOS, Congenital high airway obstructions)의 범주에 포함되며 이는 이환된 태아의 초음파 소견이 매우 유사하기 때문이다.
발생 빈도는 아직 잘 알려져 있지 않으며, 문헌에 의하면 전 세계적으로도 100예 미만이 보고되었으며 이 중 산전 진단된 경우는 많지 않다. 국내에서는 아직까지 문헌으로 보고된 적이 없으며, 쌍태임신 특히 단일융모막성 쌍태임신과 동반된 경우는 전세계적으로도 드물다. 쌍태임신에서 일측 태아의 기형이 있을 경우 적절한 치료를 선택하는데 있어 많은 어려움이 따르며, 치료의 선택에 있어 융모막성의 결정이 매우 중요하다.
저자들은 국내 및 국외에서도 단일융모막성 쌍태임신에서 선천성 후두 폐색이 보고된 예가 드물어 간단한 문헌 고찰과 함께 보고하는 바이다.

Congenital laryngeal atresia is an extremely rare anomaly that if undiagnosed prenatally, almost always causes to death within several minutes after birth due to respiratory obstruction.
This anomaly, with tracheal atresia, tracheal cyst or web is categorized into a single entity of congenital high airway obstructions (CHAOS) because ultrasonographic findings of affected fetuses are very similar. There has been less than 100 reported cases of this anomaly, but a few cases were diagnosed prenatally worldwide.
In addition, most reported cases were associated with singleton pregnancies and only one case was in twin pregnancy until now. It is dilemmatical to choice management options in a discordant anomaly in twin pregnancy, especially monochorionic twin.
We experienced the first case of congenital laryngeal atresia of one fetus in monochorionic twin in Korea. We report our case with brief review of literature.
