Congenital dacryocystocele is an uncommon variants of congenital nasolacrimal duct obstruction. Congenital obstruction of the nasolacrimal duct may result in a dacrocystocele, a cystic swelling of the nasolacrimal duct which is present at birth and may be unilateral or bilateral. Imaging is helpful to distinguish this condition from encephalocele, haemangioma or dermoid. We report a case of congenital bilateral dacryocystocele found by ultrasonography.

선천성 누낭류는 선천성 비루관 폐쇄증의 드문 형태로서 누계(lacrimal system)의 상, 하부가 동시에 폐쇄됨으로써 발생한다. 보통 출생 시 혹은 출생 후 몇 주 이내 누낭이 팽창되어 내안각 바로 아래 부위에 회청색의 낭성 종물이 나타나며 이차적으로 누낭염이 동반되는 경우가 많다. 본 증례는 임신 30 주경에 시행한 초음파 검사에서 양쪽 orbit 안쪽 아래쪽에 cystic lesion(오른쪽: 10 mm, 왼쪽: 9 mm) 의 소견보여 Level II Obstetrics ultrasound 시행한 결과 congenital bilateral dacryocystocele로 산전 진단하여 분만 후 조속히 치료할 수 있어 환아의 합병증을 감소할 수 있다.
