34세 분만력 0-0-2-0 산모가 임신 15주에 태아 기형과 자궁내 태아 사망을 주소로 내원하였다. 산모의 염색체 핵형은 정상이었고 아버지의 염색체 핵형은 46, XY,t(2;5)(p21;p15)로 균형전좌이었다. 태아 기형에 대한 초음파 검사상 크게 보이는 대뇌 조직이 붕괴되어 보였다. 3D US ((ACCUVIX XQ, 1.04 version, Medison, Seoul, Korea)으로 관찰시 머리덮개뼈의 납짝뼈는 보이지 않았고 뇌이랑 부위가 보이면서 뇌 조직이 관찰되었다. 사태아(남아) 90 g을 분만하였으며 태반 조직의 융모막 조직으로 시행한 염색체 검사상 46, XY, der(5)t(2;5)(p21;p15) pat, 부분 삼염색체증 2p와 말단 5p결손이었다.
본 저자들은 3차원 초음파로 외번뇌증을 진단하고 시행한 염색체 검사상 부측에서 기원한 부분 삼염색체증 2p와 말단 5p결손 (46, XY, der(5)t(2;5)(p21; p15) pat)를 확인하였기에 보고하는 바이다.

A 34-year-old pregnant woman, parity 0-0-2-0, was referred at 15 weeks of gestation with suspected fetal anomaly without fetal heart rate. The mother’s karyotype was normal and the father’s karyotype was 46, XY,t(2;5)(p21;p15). On prenatal ultrasound, the following anomalies were detected: cranium that characteristically involves a large, disorganized mass of cerebral tissue. The flat bones of the calvaria are absent, leaving the brain mass uncovered with convolutions or “pseudo” sulcal patterns present by 3D US ((ACCUVIX XQ, 1.04 version, Medison, Seoul, Korea). She delivered a dead male 90 g and cytogenetic analysis of the cultured placenta tissue (chorionic villi) revealed a 46, XY, der(5)t(2;5) (p21;p15)pat karyotype with partial monosomy and partial trisomy.
We have reported a case of paternally-derived partial trisomy 2p and distal 5p deletion (46, XY, der(5) t(2;5)(p21;p15)pat) with exencephaly diagnosed by 3D-US.
