치명적 난장이는 신생아의 치사성 골격계형성이상 중에서 가장 흔하다. 이 질환은 심하게 짧고 휘어진 뼈, 좁은 가슴, 이마가 돌출된 큰 두개골, 편평한 척추가 특징이다. 치명적 난장이의 산전진단은 특징적인 초음파 소견을 이용한 방법이 잘 확립되어 있으나, 골형성부전증, 연골무형성증, camptomelic 이형성증, 연골형성부전 등 사지단소증을 보이는 다른 골격계형성이상과의 감별진단은 어려울 수 있다. 치명적 난장이는 2가지 형태가 기술되었고, 둘 다 fibroblast growth factor receptor 3 gene의 새로운 상염색체 우성 돌연변이에 의해 발생하는 것으로 생각된다. 저자들은 초음파로 산전 진단된 치명적 난장이의 2가지 형을 기술하고, 문헌고찰을 통해 다른 골격계형성이상과의 감별점을 고찰해보기로 한다.

Thanatophoric dysplasia is the most common type of neonatal lethal skeletal dysplasia. This disorder is characterized by severe micromelia with bowing, narrow thorax, macrocrania with frontal bossing, and platyspondyly. The prenatal diagnosis of thanatophoric dysplasia has been well established by characteristic sonographic features, however differential diagnosis of thanatophoric dysplasia from other short-limbed skeletal dysplasias such as osteogenesis imperfecta, achondrogenesis, camptomelic dysplasia, and achondroplasia may be difficult. Two types have been described, and both are now thought to be due to new autosomal dominant mutations in the fibroblast growth factor receptor 3 gene. In this paper we described the two types of hanatophoric dysplasia diagnosed prenatally by ultrasonography and reviewed the tips of differential diagnosis from other skeletal dysplasias in the literature.
