We present a case of twin-twin transfusion syndrome, in which case, hydrops fetalis in donor twin occurred after one fetal demise and therapeutic amnioreduction at 26 weeks of gestation.
A 34 years old primiparous woman with twin pregnancy was referred because of complaints of abdominal distension and one fetal demise at 26 weeks' gestation. Ultrasonographic examination showed one fetal demise with severe polyhydramnios (recipient) and alive fetus (donor) with oligohydramnios, suggesting twin-twin transfusion syndrome. During the conservative management polyhydramnios gets worse and she complained dyspnea. We aspirated 800 ml of amnionic fluid at a time/day for 3 days from 26 week 2 day of gestation (total 3 times). At 27 weeks of gestation, the donor twin developed hydrops fetalis on ultrsonographic examination and were delivered at the next day due to preterm premature rupture of membrane.
We report a case of the donor developed hydrops fetalis due to volume overload after therapeutic amnioreduction.
