제왕절개 반흔에 발생하는 자궁벽내 임신은 상당히 드문 질환으로서, 자궁파열이나 대량출혈 등의 심각한 합병증을 유발하여 산모의 생명을 위협할 수 있는 질환이다. 치료 방법으로는 자궁적출술 및 자궁절개술을 포함한 개복술, 복강경 수술, 흡입 소파술 및 methotrexate의 전신적 혹은 국소적 주입등이 있다. 경질초음파검사를 시행하여 초기에 진단되면, 자궁과 수태력을 보존할 수 있는 치료법이 시행될 수 있다. 저자들은 경질 초음파 유도하 methotrexate 국소주입치료로 완치된 제왕절개 반흔에 발생한 자궁벽내 임신 1예를 경험하였기에 간단한 문헌고찰과 함께 보고하는 바이다.

Cesarean scar pregnancy is a rare but possibly life-threatening condition. This ectopic pregnancy may cause grave complications such as severe vaginal bleeding or spontaneous uterine rupture. Suction curettage, exploratory laparotomy, laparoscopic surgery and systemic or local injection of methotrexate are the treatment methods currently performed.
Ultrasound permits earlier and accurate diagnosis of Cesarean scar pregnancy, allowing successful preservation of the uterus.
In this report, we present a case of Cesarean scar pregnancy treated successfully with transvaginal ultrasound-guided methotrexate local injection.
