목적 : 이 연구는 임신 36주에서 38주 사이에 측정한 자궁경부 길이로 임신 41주 이전의 자연 진통을 예측할 수 있는가를 알기 위해 시작되었다.
연구대상 및 방법 : 임신 36주에서 38주 사이의 단태 임신의 산모들을 대상으로 자궁경부 길이를 질초음파로 측정하였다. 자궁경부 길이와 41주 이전의 자연 진통의 예측성과의 상관 관계는 SPSS 6.0을 이용하여 수신기작동특성곡선(ROC curve)으로 분석하였다.
결과 : 86명의 산모들을 대상으로 연구한 결과, 평균 자궁경부 길이는 25.7 mm였다. 자궁경부 길이가 20.0 mm 미만인 산모군은 임신 41주 이전에 자연 진통이 올 가능성이 현저하게 높았다. 임신 36주에서 38주 사이에 1회 측정한 자궁경부 길이가 30.6 mm 미만인 경우에 41주 이전에 자연 진통이 올것으로 예측되는 민감도는 73%, 특이도는 92%였다.
결론 : 임신 36주에서 38주 사이에 1회 측정한 자궁경부 길이는 41주 이전의 자연 진통을 예측할 수 있는 독립적 인자이다. 이러한 정보는 임신 말기 산모들을 상담하는 데 있어 큰 도움이 될 것이다.

Objective: To investigate the relationship between cervical length at 36 to 38 weeks of gestation and prediction of spontaneous onset of labor by 41 weeks of gestation.
Methods: We performed transvaginal ultrasound to measure cervical length in women with singleton gestation at 36 to 38 weeks. We used a receiver operating characteristic curve (ROC) to assess the relationship between cervical length and prediction of spontaneous onset of labor by 41weeks by SPSS 6.0.
Results: For the 86 women included in the analysis, the mean cervical length (+/-S.D) was 25.7±1.0 mm. Significant probability of spontaneous labor within 41 weeks of gestation was increased when cervical length was < 20.0 mm. A single cervical length measurement below 30.6 mm between 36 and 38 weeks of gestation predicted spontaneous onset labor before 41 weeks gestation with a sensitivity of 73%, specificity of 92%.
Conclusion: Cervical length measurement at 36-38 weeks of gestation is an independent predictor of delivery by 41 weeks.
This information can be utilized when counseling patients regarding the management of term pregnancies.
