임신기간이나 분만 중 발생하는 자궁파열은 드문 질환이긴 하나, 일단 발생하면 산모 및 태아에게 매우 위험한 질환 중 하나이다. 임신 중 발생하는 자궁파열은 일반적으로 제왕절개술 기왕력이 있는 경우에 가장 흔하며, 그 외에도 자궁근종절제술 또는 자궁근종용해술 등의 자궁수술의 기왕력이 있는 경우에도 자궁파열이 생길 수 있다는 증례들이 보고된 바 있다. 임신 기간 중 자궁근종이 발견되는 경우 제왕절개수술 빈도의 증가, 조기진통, 산후 출혈 등의 위험성을 고려하는 것이 일반적이다. 하지 만 과거 자궁의 수술경력이 없는 산모의 경우 분만 과정에서 자궁의 파열에 대한 위험성 여부는 알려 진 바가 없다. 최근 본원에서는 과거 수술 경력이 전혀 없는 산모에서 분만 과정 중 자궁근종이 있는 부위에 자연적인 자궁파열이 발생하였고, 그로 인하여 혈복강이 생긴 사례를 경험 하였기에 저자들이 경험한 1예를 간단한 문헌고찰과 함께 보고하는 바이다.

Uterine rupture during pregnancy or delivery is rare, but one of the most dangerous events to pregnant women and fetus. Uterine rupture most commonly occurs in women with history of previous cesarean delivery. And also, cases of women with surgical history of uterus such as myomectomy or myolysis have been reported. When myoma is diagnosed during pregnancy, physician need to consider about risks of preterm labor and postpartum bleeding, cesarean delivery due to obstructed obstetric canal etc. But uterine rupture by myoma in pregnancy has not yet been reported. Recently, in our hospital, we have experienced a case of spontaneous uterine rupture on the part of uterine myoma and consequently resulting in hemoperitoneum in a woman with no previous surgical history. So, we hereby present a case with a brief review of literatures.
